BMI limitations: why it's not enough

A bodybuilder can be classified as 'obese' by BMI, and an elderly person with sarcopenia as 'normal'. It is a population screening tool, not an individual diagnosis.

BMI is the most popular tool for classifying weight, but also the most misunderstood. It is a population screening tool, not an individual diagnosis.

Where BMI comes from

It was developed by Adolphe Quetelet in the 19th century for statistical studies of populations. The WHO adopted it in 1985 as a cheap epidemiological tool. It was never designed to evaluate individuals.

Its main flaws

  • It does not distinguish muscle from fat: a 95 kg bodybuilder at 1.75 m gives a BMI of 31 (Obesity Class I) with 8 % body fat.
  • It does not distinguish visceral fat from subcutaneous fat: visceral is the dangerous one.
  • It underestimates risk in older adults with sarcopenia.
  • It fails for Asian ethnicities: cardiometabolic risk appears at lower BMI (>23).

To evaluate real cardiometabolic risk, waist circumference is better than BMI: above 102 cm in men and 88 cm in women, risk skyrockets regardless of BMI.
